The Final Attack Materia will use whatever Materia it's paired with when your character dies. Depending on the rank of your Final Attack Materia the ability may be used only the first time you die or up to 5 times. This means if you pair Final Attack + Revive and Final Attack is mastered you can die 5 times during a single battle and it'll revive you each time.
I'd highly recommend leveling at least one copy of the Final Attack Materia to pair with the Phoenix Summon. This will resurrect your entire party if you get wiped out - or just that one character. The other great combination for Final Attack is Revive, I personally don't like pairing it with an offensive attack but that's my personal choice.
Where to get Final Attack Materia:
Gold Saucer - Battle Square: This is won from the special battle at the Battle Square which becomes available after winning the W-Summon Materia and Omnislash
Useful Final Attack Materia Combinations:
Final Attack + Revive: This combination will automatically use Revive on your character when they die
Final Attack + Knights of the Round: This combination is recommended online all the time but I think it's stupid. It's too long of an animation and a waste of your time
Final Attack + Phoenix: This is the best pairing for Final Attack, Phoenix will revive your whole group and full heal it
